Please confirm each of the following before proceeding. All questions are required.

1. I am 18 years of age or older.

2. I am currently located in the United States.

3. I am not currently pregnant or breastfeeding.

4. I do not have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

5. I do not have a history of pancreatitis.

6. I do not have a current diagnosis of anorexia nervosa or bulimia nervosa.
